The Rise of Carfentanil and Government Efforts to Stop It

Carfentanil, a substance originally developed for tranquilizing large animals such as elephants, has emerged as a significant danger on the black market. This synthetic opioid is considered one of the most potent drugs available, with a dose as small as a grain of salt potentially proving fatal.

Carfentanil is an extremely potent synthetic opioid derivative of fentanyl. It poses a severe threat to public safety and exacerbates the opioid crisis. Deaths involving carfentanil are increasing again, with a huge increase reported in the past few years. 

Carfentanil is often mixed with other illegal drugs or pressed into counterfeit pills that look like prescription painkillers, often without the user's knowledge.

This greatly increases the chance of a fatal overdose. The extreme potency of carfentanil makes overdose treatment difficult, requiring multiple and possibly higher doses of naloxone compared to other opioids.

Its rise in unauthorized use has drawn the scrutiny of federal investigators and prosecutors, leading to an increase in crackdowns aimed at cutting off its distribution chain and addressing its devastating impact on public health.

What Is Carfentanil?

Carfentanil, an opioid analgesic about 10,000 times more potent than morphine and 100 times stronger than fentanyl, is a substance that demands utmost caution. Its extreme potency makes it highly dangerous, even in tiny amounts, instilling a sense of fear in those who come into contact with it.

What Is Carfentanil?

While it is legally used in veterinary medicine to tranquilize large animals, it has increasingly entered illicit drug markets, often without users knowing. Carfentanil is often mixed with or substituted for heroin or fentanyl, creating a deadly combination that puts users at serious risk.

The danger lies not only in its potency but also in the ease with which it can be synthesized and smuggled. A small amount of carfentanil can produce thousands of lethal doses, making it a favored substance for illegal manufacture.

For this reason, authorities have struggled to keep it out of street supplies, as even seemingly insignificant quantities can cause widespread harm.

Carfentanil's reappearance on the black market is a relatively recent and troubling trend. This synthetic opioid emerged in illegal drug supplies in the mid-2010s, but its prevalence has surged in recent years.

According to reports from the Centers for Disease Control and Prevention (CDC), overdose deaths involving carfentanil increased sevenfold from early 2023 to early 2024, spanning dozens of states.

Notable Federal Carfentanil Cases

Some of the more notable federal carfentanil cases in recent memory include:

  • Wasilla, Alaska (2025): In June 2025, a federal grand jury indicted Sean Mobley, 45, for distributing carfentanil in two transactions in November 2024. One transaction resulted in a non-fatal overdose of an adult, and the other tragically caused a minor's fatal overdose. This marks Alaska's first-ever federal prosecution involving carfentanil. Originally misidentified as fentanyl, the substance was later confirmed through laboratory analysis.
  • Provo, Utah (2025): Clint Pendleton was indicted in January 2025 for attempting to acquire approximately 20 grams of carfentanil. Intercepted by U.S. Customs at Los Angeles International Airport, the shipment was traced to Pendleton, who allegedly tracked the package and intended to distribute the drug in Utah. Prosecutors filed charges of attempted possession with intent to distribute. This is the first federal carfentanil case filed in Utah.
  • Bronx, New York (2018): In May 2018, the DEA seized more than 100 grams of carfentanil, the first such seizure of its kind in New York. Three individuals were charged with conspiracy to distribute over 100 grams of carfentanil, with undercover DEA operations providing the basis for their indictment. Their case culminated in significant penalties, including a mandatory minimum of 10 years in prison, providing an early precedent for addressing carfentanil-related crimes harshly.

Government Efforts to Combat Carfentanil

Recognizing the unparalleled risks posed by carfentanil, federal law enforcement and prosecutors have intensified their efforts to curb its distribution.

These efforts, which include complex investigative methods, international coordination, and significant charges, are a reassurance to the public that they are supported in the fight against carfentanil.

The federal government uses a multi-faceted approach to address the opioid crisis, including efforts to combat carfentanil trafficking and support addiction treatment and research. Some efforts include the following:

  • Targeting trafficking networks. Federal law enforcement operations target and dismantle drug trafficking networks and the people involved in smuggling illegal opioids across borders.
  • Disrupt supply chains. The Department of Homeland Security (DHS) and the Drug Enforcement Administration (DEA) target drug cartels and supply chains internationally, working with foreign partners to stop illegal drugs and precursor chemicals into the United States.
  • Border seizures. Increased efforts have led to more seizures of fentanyl and related substances, especially at ports of entry.
  • Border screening. Increased efforts have made better detection of hidden fentanyl possible due to increased screening technology. 
  • Scheduling of fentanyl-related Substances (FRS). The HALT Fentanyl Act permanently classifies fentanyl-related substances as Schedule I drugs, making criminal penalties for trafficking easier to enforce, which helps police combat these dangerous substances.
  • Increased research. The HALT Fentanyl Act also streamlines the research and registration process for FRS. This aids research into these substances to understand their effects better and develop more effective treatments and reversal agents.
  • Public awareness. Campaigns and educational initiatives are crucial in the fight against carfentanil. By focusing on the dangers of synthetic opioids like carfentanil and emphasizing the risk of unintentional overdose, these initiatives empower the public with the knowledge they need to stay safe.
  • Risk reduction tools. Distribution of risk reduction tools such as naloxone, an opioid overdose reversal medication, is promoted, as is education on safe drug use practices.
  • Access to treatment. Programs seek to increase access to and retention in treatment programs for substance use disorders, including medication-assisted treatment (MAT) and behavioral therapies.
